Staff Handbook is now available online. As required by ACA, all staff must read it before orientation. Please submit required paperwork ASAP.

Health History Form

• Click here to complete your Health History form online - This is mandatory for all staff.

Other Important Documents

• Working Papers Information – please read if you will not be 18 by June 28, 2010
• Medication Release Form – anyone under 18 (as of June 28, 2010) must complete in order to be given any prescription or non-prescription medication at camp, MUST be signed by a doctor
• W-4 Form - All staff must complete and return in order to get paid.

• I-9 Form - All staff must complete and return. You must include copies of ID with the form.
• W-11 Form - Any staff who does not hold a full-time job needs to complete this form
